By the time she has got downstairs, pottered around the garden and got back up again and settled no more than 10-15 minutes

The insulin dose can only be increased in small increments over a period of time
The extra 2 units a day should be helping
It's possible her body is becoming tolerant of the Insulin, which happens or it could be something else altogether
Probably needs a full round of blood tests to check kidney function etc.
The vet wasn't overly concerned on Thursday, to the point they actually forgot to give her the insulin

Clearly needs a higher insulin dose but it has to be increased gradually and over time apparently
For her size she is still on a relatively low dose apparently
