Don't agree with the regional winners, back when they did it with the Scandinavian tour it really showed that the depth isn't there to graduate a player each year and/or the top players ended up not using the card anyway. IMHO the biggest change that's needed is to do only the first stage of Q School regionally, but the final stage combined. Top players (recent tour card holders, top X of CT / DT / regions, etc.) going directly to final stage.

With the caveat that everyone who gets a card should be required to play the majority of the events. This will make sure the regional winners don't waste their cards. If you have a medical exemption its fine but other than that this rule needed to be made anyway. 1 player a year with 4 cards means that the regional winners would only take up 8/128 places, not a big deal really.
